The purpose of this assignment is to help students understand the components of an Advanced Directive. Advanced directives can include one or more of the following documents: living will, health care surrogate, health care power of attorney, etc. These documents are used frequently by medical professionals to determine the extent of medical intervention that is wished by an individual at end-of-life.
Each student must complete an Advanced Directive. You can use any standardized form that you choose. Resources can be found below. These resources are FREE. Students are not expected to pay for any legal service or online form as part of this assignment. In Florida, most advanced directives do not have to be notarized.

Students will submit their completed advanced directive with a narrative. The narrative is expected to be 3- 5 pages long following APA guidelines. Write an analysis and reflection on which document you chose, why you chose it, and what factors you considered when completing the advanced directive.
