This template will help you organize your Case Study Analysis papers. Although your paper should be formatted in the following order, you are encouraged to capture ideas and insights as they occur to you.

Summary of Current State
Describe the situation as it now exists (current state). Try to state it in a single sentence.

Summary of Desired State
Describe how you would like to see the situation (desired state). Try to state it in a single sentence.

Problem Statement
Summarize your position in the form of a problem statement. Try to state it in a single sentence.

Driving Forces
Identify up to five (5) factors that are presently helping to move things in the desired direction (driving forces).

Restraining Forces
Identify up to five (5) factors that are presently keeping the situation in its current state, things that are holding back moving towards the desired state (restraining forces)

Diagnosis Statement
Summarize the primary causes of the problem (diagnosis). Try to state it in a single sentence.

Proof of Diagnosis

List the primary causes of the problem and note the evidence for each. Identify the fewest number of causes (up to 5) that account for the majority of the problem. Note action steps as they occur to you, don’t be concerned about the order or priority of the actions at this point. Cause Evidence Action Plan Ideas

Outcome Statement
 State the expected outcomes that your Action Steps are going to bring about. The Outcome
Statement is often created after the Action Steps have been identified, prioritized, and
sequenced.

Action Plan
 The general purpose of the action plan is to improve the current situation by acting on the
causes of the problem.
