When you read the clinical case (Attached), I want you to imagine that you are not reading it but instead you have a client/patient in front of you and he/she/they is telling you their story. Based on what you “hear” (read in the Traumatic condition clinical case), you are required to send me a clinical letter indicating what you believe to be the diagnosis and how or why you came to this conclusion. However, there is a very specific format that you must follow. The format is as follows:
You will submit a typewritten (double spaced) paper which includes-
(1) A cover letter that contains your name and student number
(2) The paper WILL have the following subheading:
(a) A referral question
(b) Your quick autobiography
(c) Your clinical opinion
(d) A point-form summary
(e) Summary and recommendations
” Note: you are allowed to create the client’s gender, name, military rank, and age (nothing else…. If it is not information that appears on the clinical case DON’T ADD it)
Under the subheading of referral question:
It is important for a professional to indicate why you have seen this patient. So, create a couple of lines as to why you have been asked to see this patient.
EXAMPLE: Thank you Dr. Lopes for referring this client to my attention. I had the opportunity to complete an assessment on Cpl. Blogins to address the questions of a possible diagnosis, suitability to return to duty, and treatment recommendations.
The above is only an example… as you can see, it is short and states why you have seen the person. You can create your own. If you are so inclined (in other words OPTIONAL) you can also add a longer bit indicating that you have addressed the issues of confidentiality with your patient and that he/she is aware that you would be completing this report.
